The Jemicy School application is available online. Print, complete, and then mail to either the Lower and Middle School or the Upper School Admission Office:

Application For Admission

All applicants complete and mail to the appropriate admission office address with their $50 application fee.

Application Checklist

Applications are due by January 15th. Applications received after the 15th will be considered as openings become available.

English Teacher Evaluation Form

Applicants from grades 6 – 12; given to your child’s current school — the teacher will complete and mail forms to Jemicy.

Math Teacher Evaluation Form

Applicants for grades 6 – 12; given to your child’s current school — the teacher will complete and mail form to Jemicy.

Teacher Evaluation Form

Applicants for grades 1 – 5; given to your child’s current school — the school will complete and mail form to Jemicy.

Release of Records Form

All applicants – to be signed by parent/guardian and given to your child’s current school.
